ACT is a mobile, multi-disciplinary, mental health team. They bring support services to the individual, instead of the individual seeking out the services. ACT works with individuals with severe mental illness, whose needs have not been well met by more traditional approaches.

This program is open to those age 18 and older.

 

ACT Team services Include

• Help and support 24 hours a day/seven days a week

• ACT teams have primary responsibility for all services. As a multidisciplinary team, rather than refer individuals to other programs and services, the team provides the treatment and services needed.

• The staff works with individuals in their homes, neighborhoods, work other places where problems arise.

• ACT works closely with each consumer to develop an individual plan that is reviewed daily by all involved staff members.

Treatment is covered by Medicaid, Medicare, and most health insurance plans. Oswego Health Behavioral Services works closely with managed care companies and governmental agencies to communicate patient progress and facilitate appropriate discharge planning.
