Helping you with your discharge planning needs are staff members from Oswego Hospital’s Case Management Department.
The Case Management Department will work with you, your family and physician in planning your care following a hospital stay. Staff members will provide you with information on Medicare, Medicaid, nursing home placement, and can also provide Health Care Proxy and Living Will documents. For your convenience, a Notary Public is on staff.
If you have any problems while in the hospital, concerns about leaving, questions about your care, or any special circumstances, ask your nurse to call the Case Management Department.
In addition to the assistance available through the Case Management Department, other hospital staff is available to assist you with any problems or concerns. Ask your nurse, or the Clinical Nurse Manager responsible for your hospital floor to assist you in obtaining help.
